Organizations seeking funding through United Way of Sampson County should complete the required forms and submit all materials listed in the application checklist below. Download the application documents, complete each form, and include all required supporting materials.
Current Funded Agencies
Complete Application Checklist
Financial Records attached in accordance with Audit Policy
List the board of directors for your organization including addresses, phone numbers, and email if available.
Funding Application
Agency Summary Form
Audit or most recent IRS 990 or if N/A the last 6 months of bank statements including income statement and balance sheet.
New Agencies
that have not been previously funded INCLUDE ALL ABOVE PARTS plus:

NC Department of Revenue Tax Exemption Letter
NC Solicitation letter or exemption letter
Agency, Programming, Advertising, Marketing brochures and materials.
Agency By-Laws
Board Approved Policy concerning non-discrimination signed by the Board President
